Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code Annual/Monitor Inspection on July 6, 2023. There were 161 children ages 6 weeks to 13 years old present under the direct care of 33 staff. The Licensing Inspector reviewed 10 children?s records and 11 staff records which include transportation records for 1 staff, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on inspection, the Center failed to ensure each building that was built before 2015 and that houses a child day program that serves preschool-age children is equipped with at least one carbon monoxide detector.

EVIDENCE:
A carbon monoxide detector is not installed.
Plan of Correction: Maintenance/building officials will install carbon monoxide detectors in each building.

Based on records review, the Center failed to complete Child Protective Services Background Check for each staff within 30 days from the date of hire.

EVIDENCE:
Licensing Inspector review 11 staff records. The record for Staff #5 did not include documentation of an initial CPS Background Check. The date of hire for Staff #5 is 10/8/2019.
Plan of Correction: The Center Director must send documentation to the Licensing Inspector that the background checks have been requested no later than 10 days following this notification date 7/06/2023
