Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 04/28/2021 and concluded on 04/29/2021. The director was contacted by telephone to initiate the inspection. There were 21 children present and 3 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update Course sponsored by the Department of Social Services by March 1, 2020 and annually thereafter.

Evidence:
1. The record of staff #1 (DOH:04/14/2008) contained documentation of the Virginia Annual Health and Safety Update dated 03/05/2020.
2. The record of staff #2 (DOH:01/15/2013) contained documentation of the Virginia Annual Health and Safety Update dated 10/14/2020.
3. The record of staff #3 (DOH: 06/22/2010) contained documentation of the Virginia Annual Health and Safety Update dated 11/04/2020.
4. The vendor acknowledged that the training update was not completed by 03/01/2020 as required.
Plan of Correction: Staff #3 transferred to West with this update completed. Staff #3 had to complete it immediately. Updated courses were completed by staff during global emergency. Admin will ensure yearly updates will continue ongoing yearly. Courses were all complete just not by 03/01.
