Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on April 22, 2025. The on-site inspection began at 10:45am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 26 children present and 5 staff. The inspector reviewed 5 children?s records, 5 staff records and 5 agent records on-site. This inspection included document review, a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 28, 2025. A POC submitted after this date will not appear on the public website.

Annual approval from the health department shall be provided for meeting the requirements of food service.

The center did not have a current Environmental Sanitation Inspection.

When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed.

Medication for Child A was onsite, but the medication authorization had expired over 22 days ago. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed, and the medication was not returned or disposed of as required.

The center shall maintain written records of children?s injuries that contain all of the required information.

Three injury reports dated 03/06/2025, and 03/12/2025 did not document the date, time, and how parents were notified of the injury. Three injury reports were missing required signatures.
