Inspector Notes
The licensing inspector conducted an unannounced monitoring inspection on Monday, August 22, 2022 from 10:50am to approximately 2:15pm. There were a total of 130 children in care in the direct care of 22 staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility including all classrooms, the cafeteria, the gym, hallways, the playground and bathrooms were inspected. The required postings were reviewed and found to be in compliance. The center is equipped with toys and supplies and items were available to the children. Medication is administered when required and medication and medication authorizations were reviewed. Ten children's records and ten staff records were reviewed. Additional documentation was later submitted by the facility virtually.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
5Evidence: 1) The fingerprint results in the record for Staff #2, hired on 08/22/22, were dated 08/24/22. 2) During interview, a member of management confirmed the fingerprint results for Staff #2 were not obtained prior to hire.
Evidence: 1) The record for Staff #9, hired on 08/01/22, indicated the staff had resided in a
state outside of Virginia within the last five years. The record did not contain a search of a criminal history record information check from the state. 2) During interview, a member of
management confirmed the required out-of-state check was not obtained for Staff #9 within
the required time frame. The out-of-state criminal history record information check must be obtained prior to hire.
Evidence: 1) The TB screening in the record for Staff #2, hired on 08/22/22, was dated 08/23/22. 2) The TB screening in the record for Staff #3, hired on 08/22/22, was dated 08/23/22. 3) The TB screening in the record for Staff #9, hired on 08/01/22, was dated 08/02/22. 4) During interview, a member of management confirmed the TB screenings for Staff #2, Staff #3, and Staff #9 were not completed within the required time frame. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
