During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 5/19/2025 and completed on 5/20/2025 in response to a complaint received by the licensing office on 5/13/2025 relating to allegations of care and protection. The inspector reviewed zero children?s records and four staff records on-site 5/20/2025. This inspection included document review, tour of the facility, interviews, observations and measurements.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Though not a part of the original complaint, violations that were not reported were found related to staff-to-children ratio requirements and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/9/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 1/29/2025, a large cutting knife, was left sitting on the kitchen serving counter, within reach and access to children who were present in the snack area.
Plan of Correction: The serving counter is an area located inside of the kitchen and not the cafeteria and is off limits/not accessible to students. Educators use this area to cut fruit for afternoon snack so it is fresh to serve students. Students are not permitted to grab items off of the counter. In order to streamline any concerns, our cook will prepare the afternoon fruit and store it in the refrigerator for the classes. This will eliminate the need for cutting utensils to be out in the kitchen.

The required ratio for children of the age two years old is one staff per eight children.

On 4/30/2025, Staff #3, asked Staff #4 if she could supervise her class, Classroom B. Staff #4 left, Classroom A, which consisted of 12 children, ages two-years-old, leaving Classroom A out of ratio. The required ratio for two year olds is 1:8.
Plan of Correction: Out Two's Team consists of 10 full-time staff members. The team was reminded of the required ratio and to utilize their walkie-talkie if they are needed to leave their room for any reason.

Date corrected: As soon as another staff member saw that there was a 12:1 ratio in the classroom, they stepped in to help with coverage so correction was within 2 minutes.
