Inspector Notes

An unannounced, on-site renewal inspection was initiated on 09/09/2024 and completed on 09/09/2024. The on-site inspection began at 11:40 am and ended at 2:30 PM. The inspector reviewed compliance in the areas listed above. There were 104 children present and 18 staff. The inspector reviewed 10 children?s records and 9 staff records on-site. This inspection included: ? document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each staff member shall submit documentation of a negative tuberculosis screening and the documentation shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
The screening for Staff 3 was completed 414 days prior to date of employment.
Plan of Correction: Administration will not accept tuberculosis screening over 30 days prior to date of employment.
