Each center shall maintain and keep at the center a separate record for each child enrolled which contains the following: place of employment and work phone number for each parent who has custody.
Two out of five child records reviewed did not contain complete employment information for each parent. On the date of the inspection, Child #1 had been enrolled for six months. Child #2 had been enrolled for four months.

Plan of Correction: When parents leave employment lines blank due to self employment, or can only provide cell not work number, they will be asked to write NA on form. The 2 parents in question were contacted on 1/8/2026 to confirm NA should be written on form.
