Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 4/13/21 and concluded on 4/27/21. The Director was contacted by email to initiate the inspection. There were 139 children present with 37 staff. The inspector emailed the Director a list of items required to complete the inspection. 5 children?s records and 5 staff records submitted by the facility were reviewed to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Based on file review, 1 out of 5 staff records does not have all required documentation.
Evidence
1. Staff #2 does not have documentation that two or more references as to character and reputation as well as competency were checked before employment began.
Plan of Correction: The Director will ask staff #2 to provide a letter of recommendation to keep on file.
Standard 63.2(17)-1720.1-B-4
Based on file review, 3 out of 5 staff records do not have the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence
1. Staff #2, #3 and #5 do not have a sex offender registry check on file for states they have lived in within the past five years.
2. Staff #5 does not have a criminal history record check for a state she has lived in within the past five years.
