During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 9/22/25 from 10:10 am to 12:40pm. The inspector reviewed compliance in the areas listed above. There were 135 children present and 26 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
31. Staff # 1 has been employed at the center for 2 months. Staff # 1 has resided outside of Virginia within the last five years. A CRC for Staff # 1 was not submitted prior to the first date of employment.
2. Staff # 2 began employment one month ago and one week ago. Staff # 2's has resided outside of Virginia within the last five years. A CRC for Staff # 2 was not submitted prior to the first date of employment.
1. The TB screening for Staff # 1 was dated almost two months before the first day of employment.
2. The TB screening for Staff # 2 was dated one day after the first day of employment.
3. The TB screening for Staff # 3 was dated one month and ten days after the first day of employment.
1. The center's license permits care for children from 2.5 years to 6 years of age.
2. Two children under the age of 2 years were enrolled in the M101 class and were present during the inspection. Both children started care on 9/4/25.
3. Child # 1 turned 2 years old three days after the first day of enrollment.
4. Child # 2 was still not two years old on the day of the inspection.
