During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on April 23, 2025 and completed on April 23, 2025, as a part of the current licensure period. The on-site inspection began at 10:20am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 108 children present and 20 staff. The inspector reviewed 10 child files and 5 staff files on-site. This inspection included: document review, postings review, tour of the facility and playgrounds, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Outlet in the preschool class not covered.

Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom provided that there is no second entry-way to the bathroom.

The Pre-K teacher was observed allowing children to use the bathroom without sight and sound supervision. The bathroom was across the classroom (>20 feet), the teacher asked for confirmation that no other children were in the bathroom. The bathroom also had a second door to another classroom.
Plan of Correction: The teacher will be retrained on proper care and supervision while children are using the bathroom.
