Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on May 16, 2025, as a part of the licensure period. The on-site inspection began at 10:00amand ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 80 children present and 18 staff. The inspector reviewed 10 children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the sex offender registry for Staff #1 prior to employment and who has been employed for four months and was working with children at the time of the inspection.
Plan of Correction: At the time of the inspection, the director requested a sex offender search for Staff #1. The sex offender search provided not results for Staff #1. Moving forward, management will ensure that if a staff person has lived outside the state of Virginia in the preceding five years, they will conduct a sex offender search for that state prior to employment.
