Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

The Licensing Inspector conducted an investigation in response to a self report incident of forbidden actions on 11/15/2021. Interviews were conducted with staff. The information gathered during this investigation does not support the allegations; violations were discussed with the center director.

Standard 22VAC40-185-550-M
Based on interview, the center did not maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
Evidence:
1. Two staff stated a written record of child #1's minor injury on November 15th was not made the day of occurrence.
Plan of Correction: In this instance, the Center provided the parent verbal notice of the child's injury.

The Center has issued a written reminder to its staff that an Accident Report must be completed whenever a child is injured. The report is to have staff and parent signatures or two staff signatures, with the original given to the parent and a copy maintained by the Center.
