Inspector Notes
An unannounced monitoring inspection was conducted on 4/26/2024 from 10:00am to 12:53pm. There were 192 children (2-5yrs) directly supervised by 40 staff. The physical plant, 9 staff records, 10 children?s records, 7 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, preparing for and eating lunch. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 5 business days from 5/3/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). If a plan of correction is not received within the required timeframe, it will not appear on the public website.
Violations
4Evidence: observed a spray bottle with soap water was accessible to children on sink counter
reach after each use and to remind volunteers of this rule. A ?return to storage after use? label will be added to the cleaning solutions, and a ?Store & Lock cleaning solutions after use? will be posted on the designated storage space.
Evidence: observed sunscreen accessible to children in an unlocked cabinet under the diaper changing table in room 157.
Evidence: observed in room 152 a lunch box that was not labeled with a name or a date.
A sign reminding parents and teachers of this requirement will be posted at the
classrooms? entrance.
Evidence: reviewed 10 children's records and found that:
1) Child 5 did not have documentation of authorized pick-ups
2) Child 3 did not have documentation of addresses for either emergency contact
3) Child 5 did not have documentation of emergency contacts
4) Child 2 had documented a parent as an emergency contact
The Center will ask parents to write down N/A in the corresponding form when a list
of authorized pick-up people cannot be provided.
Emergency Contacts: The Center will explain to parents who do not wish to provide emergency contacts other than themselves, that per VA Licensing requirements, their child cannot attend school until they provide at least two emergency contacts different from themselves.
