Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A monitoring inspection was initiated on 09/28/2021 and concluded on 09/30/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 23 children present, ranging in ages from 2 years to 5 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
2Evidence:
1) On the date of inspection, an emergency medication for Child #4 was available in the center. An authorization form for this medication was not available for review.
2) The center's director stated that they could not locate the authorization form.

1)The record for Child #3 did not contain the work phone number and place of employment of a second parent.
