Inspector Notes

An unannounced monitoring inspection was conducted October 25, 2023 approximately 1:25 pm -2:45pm. There were 27 children directly supervised by 8 staff. The physical plant, 5 staff records, 3 children?s records, medications on-site were reviewed, evacuation drills were inspected.

Children were observed resting. There was an assortment of age appropriate materials for the children in care.

There was an adequate number of staff with current certification CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized.

Based on review of three children's records, the center did not obtain documentation of immunizations for a child prior to the child's first attendance.
Evidence:
1. Child #2 (date of enrollment 9/1/2023) did not have documentation of immunizations for a child prior to the child's first attendance.
Plan of Correction: I will ask parents for the child's current immunization record and send you a copy.
Standard 8VAC20-780-140-A
Based on review of three children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance.
Evidence:
1. Child #2 (date of enrollment 8/20/2021) did not have documentation of a physical in record prior to or within 30 days of first attendance.
Plan of Correction: I will ask the parents for the child's current physical exam and will send you a copy.
Standard 8VAC20-780-245-J-1
Based on review of two staff records, staff interviews, and review of attendance record, the center did not have staff on duty who are medication administration training (MAT) certified to administer medication to children in care.
Evidence:
1. There are two enrolled children who have emergency medications on-site.
2.The center did not have documentation of staff on duty who are medication administration training (MAT) certified to administer medication to children in care.
Plan of Correction: The teachers will renew their MAT certifications on November 2nd and I will send you the new certifications.
