Inspector Notes

A renewal inspection was conducted on 10/25/2022 between the hours of approximately 09:45 am and 2:45 pm. There were 180 children present, ranging in ages from 2 months to 5 years old, with 54 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 5 staff records and 5 agents records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, new board agents/officers, did not have in their records completed VA criminal history record search and/or out-of-state Child Abuse and Neglect search by the end of the 30th day of the change.

Evidence:

New board officers/agents started with ACCA on 07/01/2022.

1) Agents did not have in their records, the results of an out-of-State Child Abuse and Neglect search.
* For Agent #1, it was requested on October, 2022.
* For Agent #4, there was no documentation in their record that a search has been requested.

2) Fingerprint-based criminal history check determination letters were dated after the 30th date: Agent #1- 10/05/2022, Agent #2- 08/24/2022, Agent #3- 10/19/2022 and Agent #4- 10/19/2022.
Plan of Correction: New board agents/officers will complete Sworn statement or affirmation and/or search of VA Central Registry within 30 days of joining program. AS for the cited agents/officers, their records and/or searches have been submitted and are pending governmental response.

2) No VA Central Registry search results in the record by the 30th day: Agent #1- requested on 10/07/22. Agent #3- requested on 10/09/2022, Agent #4- requested on 10/19/2022. The VA Central Registry search results in the record for Agent #2 were dated 09/16/2022.
