A monitoring inspection was conducted on 03/15/2023 to investigate a self-reported incident received by the department involving the area of forbidden actions. On this date, interviews were conducted with staff, documents were reviewed and staff/child records were reviewed. The inspector also reviewed video footage provided by the center.
