Inspector Notes

An unannounced monitoring inspection was initiated and completed on 03/06/2026, in response to a self-report received by the licensing office on 02/25/2026 relating to standards 8 VAC 20-780-340.A and 8 VAC 20-780-40.E. The inspector reviewed (1) staff record on-site on 03/06/2026.

This inspection included: document review (i.e. Policies and procedures), tour of the facility, interviews, and observations (applicable pages of the Employee Orientation Handbook)

Information gathered during the inspection determined two (2) non-compliances with the applicable standards. These violations were documented on the violation notice issued to the Child Day Center.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 02/25/2026, at approximately 3:30 pm, Child #1 was given the wrong bottle of breast milk. The bottle given was labeled with the name of Child #2.
Plan of Correction: Staff #1 was place on administrative leave. All applicable staff were retrained on name-to-face verification while feeding infants.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

On page 52 of the Employee Orientation Handbook, it states: "Human milk from a mother shall be used only with that mother's own child." Staff #1 stated that while two (2) identical bottles sat side by side, the label on the given bottle was not checked prior to feeding Child #1.
