During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on September 18 2025 and completed on October 8 2025, in response to a self-report relating to supervision. The on-site inspection began at 10:00am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 121 children present and 17 staff. The inspector reviewed 2 children?s records and 1 staff records. This inspection included a document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on October 21 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On September 11 2025, Staff #1 did not ensure the care, protection, and guidance of Child #1 (age 4 years) and Child # 2 (age 4 years) when Staff #1 left Child #1 and #2 on the playground unsupervised for approximately 10 minutes.
Plan of Correction: Staff will have updated placement on the playground to ensure the entire area can
be better monitored. In addition to name to face, staff will perform a sweep of the
grounds to ensure all children are accounted for.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On September 11, 2025, Child #1 (age 4 years) and Child #2 (age 4 years) were not within sight and sound supervision for approximately 10 minutes. Staff #1 returned with their class from the playground and Child #1 and Child #2 were left outside. Staff #1 realized that Child #1 and Child #2 were not with their group following a name to face conducted in the classroom. Child #1 and Child #2 were located on the playground by Staff #2.
Plan of Correction: Staff will follow policies already in place around supervision, adding a secondary location on the playground which places staff in a more centralized spot for better
supervision.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures.

The center's own policies for identifying where attending children are at all times, including procedures to ensure that all children are accounted for was not followed on September 11 2025.

The center's policy states that ?Regular and accurate name-to-face headcounts must be completed by staff at every threshold.?

Staff #1 did not complete a name to face check at the threshold entrance of the playground, on September 11 2025. Child #1 (age 4 years) and Child #2 (age 4 years) were left on the playground while Staff #1 brought the remainder of the children indoors. Staff #2 located Child #1 and Child #2 approximately 10 minutes after the group returned inside.
Plan of Correction: Staff shall complete name to face when both entering exiting the building and entering and then once again after arriving at their destination, whether that be another room or simply the snack table. This is already our policy and it was not followed the day in question.

Secondly, any other staff present must step in to assist or excuse themselves from the area to limit distraction.
