Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on August 2, 2021 and concluded on August 4, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 74 children present, ranging in ages from 16 months to 12 years with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of four staff records, one does not contain the required follow up tuberculosis screening.
Evidence
The record for staff number one does not contain a current tuberculosis screening report. Current report is expired.
Plan of Correction: We will make sure to ensure all employees have their updated tuberculosis screening report before their current one expires. This employee will have their updated tuberculosis screening by next week.

Evidence
The record for child number one does not contain documentation that proof of birth was obtained.
Plan of Correction: We will make sure to obtain proof of birth before a child's first day. We received a copy of this proof of birth on 8/5/21
