Inspector Notes

A monitoring inspection was conducted on January 4, 2023 regarding a self reported incident that occurred in December 2022. The inspector reviewed compliance in the areas of administration, supervision, programming, and emergencies. Staff were interviewed, video footage and records were reviewed .Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-770-60-C-2
Based on review of staff records, the child day program did not obtain a central registry finding within 30 days of employment for a staff member.
Evidence
The child day program did not obtain a central registry finding within 30 days of employment for staff #1, first day of employment 9/13/2021. Central registry was mailed on 9/18/2022, and results were received 01/05/2023.
Plan of Correction: All background checks will be submitted within the required timeframe. CRS will be sent in within 10 days of first day of employment.
Standard 8VAC20-780-550-P
Based on review of incident/accident reports, the center did maintain a complete written record for all children's serious and minor injuries.
Evidence
On December 12, 2022, child #1 had two incidents occur that required the center to complete a written record. The first incident had a written report that did not contain two signatures; either one staff and a parent or two staff members.
The second incident did not have a written report completed.
Plan of Correction: I did not realize the incident/accident reports needed two signatures. We will make sure to always get two signatures.
I will make sure when ever there is an incident with a child we will complete an incident/accident report and give it to the parent the same day.
