Inspector Notes

An unannounced monitoring inspection was conducted. Observed children in multiple classrooms with direct care staff in ratio for supervision. Children were reading stories, transitioning to PM snack, diapering and having circle time. Reviewed 2 staff records, medications on-site with children's records and authorizations, physical space and trainings. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-270-A
Based on observation, areas of the center are not maintained in clean, safe or operable condition.
Evidence:
1. The girls hall bathroom had a broken tp holder and broken soap dispenser.
2. The School-Age room had peeling paint under the "book of the week' bulletin board.
3. The small red couch in the 4/5 classroom was splitting.
4. Peeling paint in the Toddler classroom on the far wall.
Plan of Correction: Soap dispenser ordered 1/9/24.
Work order place for paint. 1/12/24
Couch has been replaced. 1/9/24
Standard 8VAC20-780-280-B
Based on observation, hazardous materials were unlocked.
Evidence:
The cabinet in the boys hall bathroom was unlocked and contained two spray bottles of cleaners and in the girls hall bathroom one cabinet was unlocked and contained a bottle of cleaning solution.
Plan of Correction: The cabinets had locks placed on them during the inspection.

Based on staff statements, in the grouping of teachers in the 4/5's classroom, one lead teacher supervised more than two aides.
Evidence:
The Lead Teacher supervises 4 aides in the 4/5's classroom.
Plan of Correction: The problem has been corrected there are two lead teachers.

Based on observation and staff statements, the maximum group size is exceeded in the 4/5's classroom.
Evidence:
The classroom had 34 children present and no primary caregivers identified for the children.
Plan of Correction: We split the classroom up.

Based on observation, an over the counter diaper cream was not labeled with child's name.

Evidence:
One OTC diaper cream in the Toddler classroom was missing the child's name.
Plan of Correction: Name was added during the inspection.

Based on staff statement and record review, documentation of emergency drills was not complete.
Evidence:
For the yea2023 the drill log did not include documentation of a shelter-in place drill as well as a lockdown drill.
Plan of Correction: WE sent evidence of the drills that they were done.
