Inspector Notes
An unannounced inspection was initiated and completed on 7/10/2025 in response to a complaint received by the licensing office on 7/8/2025 relating to allegation of supervision and behavior management. The inspection included: interviews, observations and a review of child and staff records.
The preponderance of evidence gathered during the investigation does support the allegation, therefore the complaint is determined valid. The information gathered during the inspection determined a violation with applicable standards or law. A violation was issued on the violation notice.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
1.. Child A went home on 7/8/2025 with undocumented scratches on their arms.
2. Staff #1 stated that at lunch time Child A told them that Child B pushed them. Staff #1 did not see any marks on Child A at that time.
