Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on April 17, 2021 and concluded on May 24, 2021 . A complaint was received by the department regarding allegations in the areas of staffing and supervision. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation did not support the allegations of non-compliance with standards or law. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Violations
1Evidence:
1. On 04/15/2021, Child # 1 was scratched in the face by Child #2. An Incident/Accident Report for Parent/Guardian was written for Child #2 on 04/15/2021, but not one for Child #1.
2. Documentation written by Staff #2 indicates a conversation about the scratch with the parent of Child #1, but no Incident/Accident Report was given to the parent.
3. On 04/20/21, Child #1 was scratched on the neck by Child #2. The scratch left red marks on Child #1?s neck. On 04/21/2021, an Incident/Accident Report for Parent/Guardian was written.
4. Staff #1 confirmed that an Incident/Accident Report for Parent/Guardian was not written the day of the incident.
