Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including staff records, documents review, and staff interviews.
An investigation in response to a self-reported incident was initiated on August 4, 2021 and concluded on August 30, 2021. The self-report received by the department included allegations of behavioral guidance not being constructive in nature, age and stage appropriate and intended to redirect children to appropriate behaviors.
The evidence gathered during the investigation supported non-compliance with standards or law. Any additional violations related to the complaint but identified during the course of the investigation can be found on the violation notice.

Violations
1Evidence:
1. According to an Incident Investigation Form dated 07/29/2021 for Staff #1, Staff #1 grabbed Child #1?s(4 years old) wrist and pulled them to the door. Staff #1 stated that he ?guided wrist to the door. [Child #1] was on their cot, [Staff #1] grabbed their wrist and pulled them to the door. If you are not going to listen, going to redirect to a different environment. This is your last chance or we can go sit."
2. According to a parent interview on August 3, 2021, Child #1 was upset and crying because Staff #1 was rude to them during the day. Child #1 stated that ?Staff #1 grabbed their wrist and pulled them in line.?
