Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including staff records, documents review, and staff interviews.

An investigation in response into a self reported incident was initiated on November 9, 2021 and concluded on November 10, 2021. The self report received by the department included allegations of forbidden actions. The director was contacted by telephone to conduct the investigation. The director and staff were interviewed.

The preponderance of the evidence gathered during the investigation does not validate the self-reported incident of forbidden actions. Therefore, the self report is determined to be not valid. There are no violations resulting from this inspection.
