Inspector Notes

An unannounced renewal inspection was conducted on 10/18/2023 from 9:45 am-12:53 pm. There were 42 children supervised by 9 direct care staff. The physical plant, 5 children records, 4 staff records, evacuation drills, medications, and policies were inspected. Children were observed participating in group play, tabletop activities, and playing outside. Areas of non-compliance are identified in the Violation Notice. Based on record review and staff interview, 1 of 5 children's records indicated the center failed to ensure that all physical examination records were obtained before the child's attendance or within 30 days after attendance.

Evidence: There was no documentation of a physical examination for Child #3 whose first date of attendance is 3/31/2019.
Plan of Correction: Ensure all children have physical examination by first 30 days.

Based on record review and staff interview, 1 of 4 staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence: Staff #3 (DOH: 7/19/2023) had not completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
