Inspector Notes

A self-reported investigation concerning injury reporting was initiated on 6/18/2024, from a report received by the Fairfax County Licensing office on 6/14/2024. The investigation was conducted on 6/18/2024, in the morning between 11:15 am and 11:45 am, and completed on 6/26/2024. The investigation included interviews with the administration and staff. Based on the preponderance of evidence gathered violations were not cited for injury reporting. Non-related violations that were not part of the self-report were cited during the investigation. Based on record review and staff inteview, centers operating five or more hours per day failed to have a designated rest period for at least one hour but no more than two hours.

Evidence:
1. The toddler classroom's rest period is documented on their daily schedule as 12:30 pm to 2:30 pm. However, on June 13, 2024, 7 out of 10 students exceeded the two-hour rest period.

2. According to a staff member, the rest period on June 13, 2024, began between 11:30 am and 11:45 am and ended at 3 pm, when Staff #1 arrived.

3. A staff member stated that the reason for the earlier rest period is that lunch is now served earlier.
Plan of Correction: Sleep at rest period posted for classroom 2 hr nap time.
