Inspector Notes

An unannounced monitoring inspection was conducted today from 11:00am-12:35pm. There were 132 children (infants-5yrs.) directly supervised by 23 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing with blocks, reading books, playing outside, and eating lunch. Hand-washing procedures were also observed. There was an adequate supply of materials for the children. The center was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, staff records did not all contain required information.

Evidence:
1.Staff #1's record was missing an emergency contact.
2.Staff #5's record was missing 1 of 2 required references as to their character and reputation, as well as competency required prior to the first day of employment.
Plan of Correction: Staff filled out emergency contact information. Administration will ask for emergency contact forms to be completed prior to employees first day.
Contacted a 3rd reference for missing reference check. Administration will ask for more than 2 reference contacts.
