Inspector Notes
A monitoring inspection was conducted on 11/29/2022 between the hours of approximately 10:45 am and 1:15 pm. There were 29 children present, ranging in ages from 6 months to 4 years, with 6 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
5Evidence:
1) The documentation shall have been completed within the last 30 calendar days of the date of employment. The Tb screening form in Staff #3's record (start date 06/16/2022) was dated 1/10/2022, more than 30 calendar days of the date of employment.
Evidence:
1) A current Tb screening was due for Staff #2. The Tb screening form in the record for Staff #2 was dated 07/09/2020.
Evidence:
1) On the date of inspection, the light fixture in the girls' bathroom was not working resulting in the bathroom not having proper illumination.
Evidence:
1) In the date of inspection, in the infant room, the lower compartment of the diaper changing table was not locked. Inside of this compartment, the inspector observed cleaning products and diaper creams.
2) One staff person in the room stated that the lock that they were using was broken.
Evidence:
1) On the date of inspection, inspection findings report that was posted was dated 01/27/2022. The most recent inspection of the facility was conducted on 06/06/2022.
