Inspector Notes

The licensing inspector conducted an unannounced investigation in response to a self-reported concern regarding supervision was received by the Office of Child Care Health and Safety on August 8, 2023. The licensing inspector was on site September 6, 2023, from approximately 1:30pm until 2:15pm. Interviews with staff members were completed, observations were made, and documentation was reviewed.

The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and a violation was issued.

Based on observations and interviews, the facility did not ensure appropriate supervision of all children in care, including daily intake and dismissal procedures to ensure safety of children.

Evidence:
1. On July 31, 2023, at approximately 1:00pm, it was determined Staff #1 stepped away from the doorway of the hallway bathroom while Child A (four years of age) was in the bathroom.
2. Staff #2 sent Child B (four years of age) to the hallway bathroom, not realizing Staff #1 had walked away.
3. Staff confirmed Child A and Child B were unsupervised the in the bathroom for an estimated one to three minutes.
4.The TCCC Orientation and Training form stated ?Classrooms are under sight and sound supervision at all times. This is to ensure proper supervision and that children are accompanied when needing to use the restroom or move to another area of the center. Children are under sight and sound supervision at all time.?
5. Staff #4 confirmed the policy is for staff to accompany children when needing to use the restroom and children are to be under sight and sound supervision at all times.
