Inspector Notes

A monitoring inspection was initiated and concluded on 1/30/2024.There were 81 children present with 13 staff supervising and 3 administrators. The children were observed during classroom time engaged in various centers, outside time, and lunch time. A total of 5 child records and 5 staff records were reviewed. The inspection started at 10:50 am and concluded at 1:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview the center did not ensure to obtain background checks every five years.
Evidence: The most recent central registry check for staff #4 was dated 9/5/2018.
Plan of Correction: New central registry search will be processed & obtained

Based on record review and staff interview the center did not ensure each child shall have a physical examination before the child?s attendance; or within 30 days after the first day of attendance.
Evidence: The director confirmed that Child #5 (start date 11/3/2019) did not have documentation of a physical examination.
