Inspector Notes
An unannounced non-mandated inspection was initiated on 1/30/2024 and concluded on 2/6/2024. The inspection was based on a self-report regarding an injury that required outside medical attention. The inspector reviewed compliance in administration, staff qualifications & training, staffing & supervision, special care provisions & emergencies, and special services. Staff and witnesses were interviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Evidence: The director confirmed that child #5 (age 4) has a diagnosed dairy allergy and on January 25th the child was given yogurt during snack time which resulted in an allergic reaction that required the administration of the child's emergency Benadryl and Auvi-Q Epipen. 911 was called, and the child was taken to the hospital.
Evidence: Staff #1, responsible for overseeing snack time, said they were not aware of child #5?s dairy allergy.
Evidence: Staff confirmed that child #5 who has a diagnosed dairy allergy, identified on their written allergy care plan, was served yogurt.
