During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/27/2025 and completed on 5/29/2025. The on-site inspection began at 10:43 am and ended at 2:10 pm. The inspector reviewed compliance in the areas listed above. There were 84 children present and 13 staff. The inspector reviewed 10 children?s records and 9 staff records on-site. This inspection include document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/6/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-A
The center is required to have employees undergo a background check every five years. The most recent central registry check for staff #8 was overdue by eight months.
Plan of Correction: Resubmitted request for completed background check.
Standard 22.1-289.057-A
The program is required to submit to VDH and implement a plan to test potable water for lead. The program did not have evidence of a testing plan submitted to VDH and/or the program did not have evidence of the implementation of the required testing plan.
Plan of Correction: 120 water has been contacted about lack of reply, plan should be submitted to VDH.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. Staff #3, employed for nine months, has not completed the training. Staff #4, employed for 4 months, has not completed the training.
Plan of Correction: Staff #3/#4 asked to complete VDOE training.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Two bottles of hand sanitizer were located in the cubby area and one can of shaving cream was located on a shelf. Each bottle contained warning labels, ?Keep out of reach of children,? were unlocked, and within reach of children.
Plan of Correction: Moved products & emailed teachers reminder about keeping products locked up & out of reach.
Standard 8VAC20-780-290-A-3
Electrical outlets shall have protective covers. There were 5 outlet covers missing in one classroom and 4 missing in another classroom.
Plan of Correction: Covered outlets with proper covers.
Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. The merry go round did not have the required six feet of a fall zone.
Plan of Correction: Moved equipment to all fall zone, 5/29/2025. Merry go round to be moved 6ft away, 8/4/2025.
Standard 8VAC20-780-550-G
Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills. There was no documentation of a 2024 lockdown drill.
Plan of Correction: Lockdown was practiced prior and information added.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all required information. The injury report for Child #2 was missing the date and time the parent was notified. Child #3 had two separate injury reports. The first one was missing the date and time the parent was notified and the second one was missing the date the parent was notified. The injury report for Child #8 was missing the date and time the parent was notified.
Plan of Correction: Emailed/contacted teachers proper procedure for injury reports with reminders of signatures/date/time.
Standard 8VAC20-780-560-F
When centers choose to provide meals or snacks they shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). The children were not given the required 1/4 cup of vegetables for lunch time.
Plan of Correction: Emailed teachers about proper portion sizes for lunch.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #7 was missing the employment phone number for one parent. The record for Child #9 was missing the employment phone number for both parents.
Plan of Correction: Parents have been contacted for both children to update information.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The center did not take attendance when children departed for car pool.
Plan of Correction: Emailed teachers about procedure for marking car line departure.
