During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/30/2026. The on-site inspection began at 12:40 pm and ended at 2:20 pm. The inspector reviewed compliance in the areas listed above. There were 79 children present and 9 staff. The inspector reviewed 7 children?s records and 5 staff records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/7/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-A
(Repeat Violation) Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #5 TB was dated 6 days after date of employment.
Plan of Correction: Will monitor TB test dates for 30 day requirement.
Standard 8VAC20-780-280-B
(Repeat Violation) Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. A can of disinfectant spray and a container of disinfectant wipes with warning labels, Keep out of reach of children, were unlocked in the Children's House 2 classroom, within reach of children. A bottle of hand soap and hand sanitizer with warning labels, Keep out of reach of children, were unlocked in the Children's House 1 classroom, within reach of children.
Plan of Correction: Items locked in cabinets reminder to lock up chemicals sent to all staff.
Standard 8VAC20-780-330-B
(Repeat Violation) Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. The sand provided under two sides of the Children's House playground structure did not meet the required depth to comply with the minimum standards.
Plan of Correction: Sand brought in and areas filled to meet required depth.
