Inspector Notes

This inspection was conducted by licensing staff both on-site and using an alternate remote protocol, including telephone contacts, documents review, interviews and a tour of the program.

A monitoring inspection was initiated on 6/22/21 and concluded on 10/22/21. A self-reported incident was received by the department regarding allegations in the area of behavior guidance. The center director was contacted by telephone to conduct the investigation. Interviews with staff were conducted both on-site and by telephone. Documentation related to the incident was also reviewed. The information gathered during the investigation determined no violations with applicable standards or law. No violations were issued.
