During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 6/10/25. The on-site inspection began at 10:25am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 107 children present and 16 staff. The inspector reviewed 10 children's records and 10 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Immunization records for child 1 were not obtained until over a year after the child began attending the center.
Plan of Correction: The center responded with the following: Center management will ensure immunization records are obtained by each child's first day of care.
Standard 8VAC20-780-140-A
Repeat Violation
The center shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no documentation of a physical examination in the record for child 1, who has been enrolled for over a year and a half.
Plan of Correction: The center responded with the following: Child 1's parent will be contacted today to obtain documentation of a physical exam. Children's records will be routinely audited to ensure they contain all the required information.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe condition. On the pre-k playground, there is a hole in the ground at the gate of the fence with exposed concrete, creating a tripping hazard for the children in care.
Plan of Correction: The center responded with the following: A work order will be placed with maintenance to fill the hole located on the playground.

The center shall implement a monthly practice evacuation drill. A practice evacuation drill was not completed during the month of April 2025.
Plan of Correction: The center responded with the following: Center management will ensure practice evacuation drills are conducted each month.
