Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The records for child 1 and child 2 do not contain the work phone number for one parent. The records for child 3 and child 4 do not contain the work phone number or place of employment for one parent. The record for child 5 does not contain the work phone number for one parent or the complete addresses for the two emergency contacts.
Plan of Correction: The center responded with the following: The missing information for each child's record will be obtained from the parent and documented in the record. Going foward, center management will review the child enrollment form at the time of enrollment to ensure all required information is documented correctly. Management will also regularly audit records to ensure children's information is up to date.
