Inspector Notes
A monitoring inspection was conducted on 06/06/2022. In addition to an in-person tour, the inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Children were observed during nap. Infants were observed sleeping in cribs and having "tummy time" with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation the center did not ensure all areas of the center, inside and outside were maintained in a safe condition.
Evidence:
1. In the hallway bathroom in the preschool building there was a missing tile exposing a rusted metal.
2. The swings in the play areas had rust on the chains.
3. There was peeling paint and rusted hardware on the large play structure.
Evidence:

Plan of Correction: The bathroom is scheduled to have some renovations and the rust will no longer be exposed and missing tile will be replaced. Rust and peeling paint will be removed.
