During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An inspection was initiated on 2/13/25 and completed on 3/18/25 in response to a self-report received by the licensing office on 1/31/25 relating to physical plant and operational responsibilities. The inspector reviewed one child's records and zero staff records electronically on 3/18/25. This inspection included document review, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/28/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards (at least six inches of mulch).
The resilient surfacing on the two- and three-year-old playground was measured at approximately one inch in height. Children were observed playing on the playground at the time of the inspector?s arrival.
Plan of Correction: Mulch has been ordered. Required measurements will be taken.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's facilities are maintained in compliance with these standards, the center's own policies and procedures.
The center?s policy states that ?all outdoor climbing equipment will have at least 6 inches of mulch underneath and around it? and ?the mulch will be replenished as needed to ensure the continuous safety of all children at the center. On 1/30/25, a serious injury of a child occurred on climbing equipment; the mulch was measured at approximately one inch. The center did not follow their playground safety procedures.
Plan of Correction: Mulch has been ordered. Required measurements will be taken.
