The record for Staff #1 (employed for two years) did not contain a sex offender check or central registry check for a state the staff member had resided in the preceding five years.

Each immunization report shall be signed by a physician, his designee, or an official of a local health department.
The immunization report for Child #1 was not signed by a physician, his designee, or an official of a local health department, which is required.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards (at least six inches of mulch).
The resilient surfacing on the two- and three-year-old playground was measured at approximately less than one inch. Children were observed playing on the playground.
Plan of Correction: Either mulch will be replenished and/or mats will be installed on fall zones.

Children's hands shall be washed with soap and running water or disposable wipes before
and after eating meals or snacks.
In a one year old classroom, after a diaper change, a child's hands were not washed, which is required.
Plan of Correction: All staff has been retrained in handwashing and diapering procedures.

Any vehicle used by the center for the transportation of children shall meet the following requirements: the vehicle shall meet the safety standards set by the Department of Motor Vehicles and shall be kept in satisfactory condition to assure the safety of children.
A vehicle used for transporting of children had an expired inspection and an expired vehicle registration on the day of the inspection.
