Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was conducted on 04/26/2021. A complaint was received by the department regarding allegations in the areas of Physical Plant. The in-charge person was contacted by telephone to conduct the investigation. The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and a violation(s) were issued.

Based on observation the center did not ensure areas and equipment of the center were maintained in a safe condition.
Evidence:
In the yellow classroom, there was a book-shelf with library books on it. A child in the classroom was observed grabbing onto the shelf to pull herself off the floor. The bookshelf was taller than the child and not secured to the wall.

Plan of Correction: The following corrective action was provided during the exit interview:
