Inspector Notes
An unannounced monitoring inspection was conducted on April 25, 2023. The inspector arrived at 10:40AM and departed at approximately 1:15PM. There were 60 children present and 11 staff members. A sample size of 11 records were reviewed which included 6 children?s records and 5 staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed indoor and outdoor free play, bathroom break, diapering, handwashing, lunch, and nap. First aid kits and emergency preparedness documents were observed. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site assistant director at the exit interview.

1. In the 2 year old classroom there was a cracked and broken toy bin, in reach of the children in care, that poses as a pinching hazard.
2. Staff #1 confirmed that the toy bin, in reach of children in care, was cracked and broken.
Evidence:
1. The 2 year old classroom had no labels or dates on the children?s sippy cups that clearly identifies the owner. There were 7 children present and 7 sippy cups.
2. Staff #1 confirmed that all 7 sippy cups in the 2 year old classroom did not have dates or labels that identifies the owner.
The center responded with the following: The center will ensure that all sippy cups are labeled and dated clearly which identifies the owner.
