A Subsidy Health and Safety Inspection (SHSI) was conducted on 11/18/2024. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Staff who work directly with children shall, in addition to preservice and orientation training annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course. Staff #1 did not have the annual health and safety update training completed for 2023. The last health and safety update training completed by Staff #2 was date June 2021.
