Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, who had been attending for two months, did not have documentation of an immunization record on file.
Plan of Correction: The facility responded: Ensure all immunization records are submitted prior to enrollment.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #2, who had been enrolled for two months.
Plan of Correction: The facility responded: Ensure physicals are completed prior to enrollment.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. On the playground there was broken pottery with sharp edges. The lid of a deck box was cracked and posed a pinch hazard. In a school age classroom there was a baseboard that was peeling away from the wall, there was crumbling drywall and plaster. In a preschool classroom there were 4 finger width holes in the wall within reach of children, and a tear on a diaper changing pad.
Plan of Correction: The facility responded:
The broken pottery and deck box were removed (2/5/26).
The baseboard, crumbling drywall and plaster will be repaired (2/23/26).
The holes will be patched and repaired (2/23/26).

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. There was unlocked disinfectant and all purpose cleaner in a preschool classroom.
Plan of Correction: The facility responded: The hazardous substances were locked away.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and includes all the required information. An injury record for Child #2 did not include the date, time and method a parent was notified of the injury.
Plan of Correction: The facility responded: Managers will audit incident reports as they receive them.
