Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including
telephone contacts, documents review, interviews (if applicable) and a virtual tour of the
program (if applicable).
A monitoring inspection was initiated on March 15, 2022 and concluded on March 16, 2022.
The director/provider was contacted by telephone and a virtual inspection was conducted. There
were 104 children present with 16 staff supervising including 2 administrative staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1Evidence: In Staff#1 record (date of hire: 07/22/16), the last dated central registry finding was 7/22/16 . In Staff #2's record (date of hire:6/18/82), the last dated central registry find was 09/07/16. Interview with staff confirmed documentation reviewed was the most recent for the required background checks.
