Inspector Notes

A monitoring inspection was completed on May 8, 2023 at the program. There were 108 children present with 21 staff supervising including administrative staff onsite. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff records and interview, the center did not ensure to obtain a copy of results of a criminal record, and a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years.

Evidence: In Staff#3's record (date of hire: 04/28/22) there were no results found for state of Florida and Washington. Interview with staff confirmed documentation for out of state searches were not available for review.
Plan of Correction: The center will ensure documentation is submitted to the different states to obtain results for criminal and central registry searches

Based on review of staff records and interview, the center did not ensure to obtain a central registry finding within 30 days of employment or volunteer service.

Evidence: In Staff#2's record (date of hire: 12/29/21), there were no central registry findings found for review. Interview with staff confirmed the documentation was not available for review.
