Violations

2
Standard 22.1-289.035-A
Based on review of records and interview, the center did not ensure to obtain background check every five years thereafter.

Evidence: In Board Member #3's record, the criminal background check was last dated 04/05/2018. Interview with staff confirmed the documentation available for review during the inspection.
Plan of Correction: The center director will have staff schedule and complete all background checks requirements.
Standard 8VAC20-780-280-B
Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the Toddler room, there was observation of disinfectant wipes with the labels "keep out of reach of children" and "caution" nearby the diapering station.
Plan of Correction: The center director will remind staff to lock all hazardous substances.
