Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated on 09/14/2021 and concluded on 09/15/2021. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 26 children present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and special care and emergencies. A total of two children's records and one staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center failed to ensure that documentation was obtained that each child has received the immunizations required by the State Board of Health.

Evidence:
1. There were no immunizations documented for child #2. Child #2 was six months old at the time of the inspection and the immunization documentation obtained at enrollment, April 2021, stated that the child would receive immunizations once eligible; no immunizations were available at that time. Documentation of immunizations have not been obtained for the child as of the date of the inspection. The child's first date of attendance was 4/5/21.
Plan of Correction: The Director of Operations will contact the parent and obtain the current documentation of immunizations.
