Inspector Notes

A renewal inspection was conducted on 1/12/2023. There were 43 children present with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined a non-compliance with applicable standards or law and the violation was documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Based upon review of children's records and staff interview, the facility has not ensured that children's records include the name, address and telephone numbers for parents and/or guardians.
Evidence:
1. The record provided for child 1 did not include any information (name, address, telephone number or custody paperwork) for the mother.
2. Staff 1 acknowledged that the record for child 1 contained only n/a for the mother.
Plan of Correction: The program director responded with the following:
The parent was immediately contacted during the inspection to obtain the required information and to explain why the information was necessary. Review of enrollment records will be discussed with the staff person responsible for intake.
